Who is Eligible for Hospice Care?
- Hospice services are available for individuals who have been diagnosed with an incurable disease with a life-limiting prognosis. The illness may be cancerous or non-cancerous in nature. Examples of non-cancerous diagnoses include: Cardiovascular, Pulmonary, Renal and Liver
diseases, Dementia, CVA, and Failure to Thrive.

- Patient resides in our eleven county service area.
