Hospice seeks to facilitate the alternative of dying at home or
in a home-like setting.
Hospice seeks neither to hasten or postpone death.
Bereavement services are
available after the death of a loved one. Community grief counseling is
available for both individuals and groups as well as in the workplace or
school.At Hospice of Hope, we go a step beyond
caring. We recognize the multifaceted needs of the patient and family
and offer genuine support with personal involvement in day-to-day
living. Living, not dying, is the driving force behind hospice care -
offering (if only for a short time) the quality of life that our
patients desire: living for each moment in the comfort of their home
whenever that is feasible; or in a nursing home or Hospice Care Center
room when necessary.
Hospice of Hope staff
surround the patient and family with resources for comfort and
encouragement, providing a network of support that extends above and
beyond professionalism.
- Registered nurses, highly
skilled in pain control management, under the direction of the
patient's personal physician, are available day and night; working
to ensure that the patient is as comfortable and pain-free as
possible.

- Medical directors
are also on staff, available for consultation and health care
direction.
Hospice pharmacists
work closely with physicians to monitor patient's needs.
Home health aides,
volunteers, social workers and chaplains also contribute to the hospice
network of support to the patient and family.
- The chaplain offers spiritual
guidance, assessing the patient's spiritual needs and taking time to
help meet those needs.

- A social worker
continually assesses the needs of the patient and family and helps
them prepare for end of life decisions.

- Home health aides
assist with bathing the patient, offering help with other personal
needs and extending support to the primary caregiver.

Volunteers,
with simple, sincere gestures and willingness to work, enhance the
quality care offered by Hospice of Hope. Shopping, running errands,
offering transportation, clerical work and fundraising are only a
few of the services volunteers provide. Drying tears, listening or
reading a favorite book to a patient are additional ways volunteers
contribute to the hospice team.

- Our
Family Bereavement Follow-up Program
offers bereavement support groups, grief counseling and support to
the patient's family for 15 months after the patient's death,
extending the care, concern and encouragement that hospice provided
throughout the patient's last days of life.
