Patient Rights and Responsibilities

Patients of Hospice of Hope, Inc. shall have the right to: 1. Choose a care provider and to communicate with those providers. To be fully informed at the time of acceptance of these rights and of all the rules and regulations governing patient responsibilities; 2. Be fully informed, at the time of admission, of services and of related charges, including any charges for services for which the patient or a private insurer may be responsible; 3. Be informed in a timely manner of any significant changes in services; 4. Be fully informed of their health condition, unless medically contraindicated, and to be afforded the opportunity to participate in the planning of the home health services, including referral to health care institutions or other agencies, and to refuse to participate in experimental research; 5. Request information about their diagnosis, prognosis and treatment, including alternatives to care and risks involved, in terms that they and their families can readily understand so that they can give their informed consent; 6. Refuse treatment to the extent permitted by law and to be informed of the medical consequences of such refusal; 7. Make advance health care treatment directives (e.g., living wills, health care treatment directives, durable power of attorney, do not resuscitate); 8. Confidential treatment of personal and medical records by third party payers, regulatory bodies and accrediting organizations; (Hospice of Hope Notice of Privacy Practices) 9. Be treated with consideration, full recognition of dignity and individuality, including privacy in treatment and in care for personal needs; 10. Be taught, and have the family taught, the treatment required, so that the patient can, to the extent possible, help himself or herself; and the family or other party designated by the patient can understand and help the patient; 11. Be admitted to services only if the agency has the ability to provide safe, professional care at the level of intensity needed. Patients have the right to reasonable continuity of care and to be referred elsewhere if denied service for any reason; 12. Review all health records pertaining to them unless it is medically contraindicated in the clinical records by the physician; 13. Be free from verbal, physical and psychological abuse and to be treated with dignity and to have person and property respected; 14. Appropriate assessment and management of pain. Patients of Hospice of Hope, Inc. also have the responsibility to: 1. Give accurate and complete health information; 2. Assist in developing and maintaining a safe environment; i.e.: animals secured, guns and lethal items out of sight during staff visits. 3. Inform Hospice of Hope if unable to keep a home care visit; 4. Participate in the development and update of his/her plan of care; 5. Request further information concerning anything not understood; 6. Notify Hospice of Hope or current caregiver if you must leave your home due to a natural disaster (flood, tornado, or other emergency.) 7. Assist your nurse with your pain management plan. Hospice of
Hope complies with the requirements of Title VI of the Civil Rights
Act of 1964 and is prohibited from making a distinction in the treatment
of patients on the ground of race, color, or national origin, in the
use of equipment, other facilities, or in the assignment of personnel
to provide services. Service recipients or other interested parties
who believe that a violation of the person's rights have occurred as
a result of actions by Hospice of Hope, may file a complaint with Kavin
Cartmell, Executive Director, Hospice of Hope, who shall act immediately
to resolve the complaint within fifteen working days, in accordance
with due process:
